Service Cordination is an individually designed intervention which provides primary assistance to the participating individual. It supports the individual by creating a service plan and assists in locating and obtaining local, state and federally-funded educational, vocational, social, medical and other helpful and available services.
Home Community Supportive Staff (HCSS) will assist the individual with their daily tasks of housecleaning, cooking, shopping, laundry, personal hygiene and any other tasks that are necessary to safely obtain independence for themselves in their own home.

Our workers redirect, assist, prompt and cue the individual on daily decisions, such as medication reminders, appointment scheduling, staying on task and organizing their daily activities so that one day they will be able to sustain independence within their own home.
Independent Living Skill Training (ILST) is individually designed to improve the ability of the individual to live as independently as possible in the community. Services include assessment, training and supervision of or assistance to an individual with issues related to:
  • self-care
  • medication management
  • task completion
  • communication skills
  • interpersonal skills
  • socialization
  • sensory/motor skills
  • mobility
  • community transportation skills
  • reduction of maladaptive behaviors
  • problem solving skills
  • money management
  • prevocational skills to maintain a household.
