Y.E.P. is a community-based, people-oriented program surrounding and encompassing the family unit while focusing in on the needs of the individual who is facing some of life's challenges. When tragedy happens we are often left with hopelessness and helplessness. YEP TBI Waiver Services will help bring life back to you within your community, creating a positive foundation for the future. YEP will address the needs of the individual accepted to the services by:

1. Gettting out of the nursing home environment and into a safe independent home.
   
2. Getting the medical treatments needed from home.
   
3. Keeping safe and helping remain safe in their own home.
4. Doing daily activities and following routines.
5. Working on safety needs, community-based activities and home-based skills to gain independence.
6. Working with a plan to help with cognitive functioning, education and training as well as psycho-social needs.
   

YEP is also a NYS Certified Food Pantry-providing the needy with nutritious meals.    

We accept donations of food, furniture, household goods, and baby items that we give back to those in need.
